Meconiumcrit as a Predictor of Subsequent Fetal Asphyxia in High Risk Pregnancy

Abstract
A clinical analysis of 222 cases of Intrauterine Fetal Death(IUFD) among 28, 173 deliveries at Yeungnam. University Hospital during 10 years from January 1986 to December 1995 was made. The incidence of IUFD was 0.79%, the age distribution of
mother
with IUFD was 20 to 38 years old, and the highest group was 26 to 30 years old(51.4%).
The parity of mother with IUFD was highest in nulliparous group(56.3%).
There were 35 cases(15.8%) which had previous history of spontaneous abortion, and only 2.3% of IUFD cases had previous history of IUFD.
The most prevalent blood type was Rh positive A(32.9%). The most prevalent gestational weeks when IUFD was detected was 37 to 40 weeks(21.6%).
The most common presentation was cephalic(75.2%), and the sex ratio of male versus female was 1.2:1, and 74.8% of IUFD was less than 2500gm in weight.
The modes of delivery were labor induction and vaginal delivery(68.5%), spontaneous delivery(18.0%), and laparotomy(11.7%).
The factors associated with IUFD were unexplained cases(44.6%), preeclampsia (20.7%), placenta abruption(8.1%), placenta previa(5.9%), congenital anomaly(5.4%), cord complication(4.1%), chorioamnionitis(3.6%), maternal illness(3.2%), and syphilis
(3.2%).
There were 83 cases(37.4%) of maternal complications, and the most common complication was intra or postpartum fever(34.9%).
The patients who had received antenatal care more than once were 48.6% of the cases.
The results of abnormal coagulation tests of IUFD were as follows : the low platelet count(<100,000/mm*) was 10.4%, the prolonged partial thromboplastine time(>50 sec). Was 2.2%, the hypofibrinogenemia(<150 mg/dl) was 4.5%, and the elevated
fibrinogen
degradation products(>40 ug/ml) was 14.0%.
